| 2008 Conference Registration (PDF) | Download, print, then complete this form. Registration is valid only when memebership dues are paid. |
| 2008 Membership Application (PDF) | Apply for a new membership or renew your membership to ANGF with this form. |
| Liability Waiver (PDF) | This form must be read and signed before the participant is allowed to take part in any ANGF events. |
| Designee Authorization (PDF) | Each child with a chaperone who is not a natural or legal guardian must complete and submit this form at the registration table. |
| Conference Committees (PDF) | A synopsis of each committee provides information about their functions and responsibilities. |
| Grievance/Complaint (PDF) | An ANGF participant (or participant's parent/guardian) who believes he or she has been harassed, discriminated against, or otherwise deprived of a right can use this form as part of resolution process. |
| Performance Application (PDF) | Groups or solo performers can complete this application and mail it along with their music and video/DVD/eclip audition. |
| Vendor Advertisement Form (PDF) | Apply for booth space at the conference and/or online web site advertising space at ANGF.org. |
| Advertisement or DVD Order Form (PDF) | Apply for ad space in the conference program and online web site, or order DVD videos of ANGF 2008! |
